Pick your filing status on the tax return from two years earlier.
Enter the Adjusted gross income from Form 1040 line 11 and tax-exempt interest from line 2a.
Leave "I have a Part D drug plan" on if you have a plan, or turn it off if you do not.
Read the Part B premium a month and the extra you pay; the brackets are from CMS, listed under Sources.
IRMAA, the income-related monthly adjustment amount, is the extra Medicare charges people with higher incomes on top of the Part B premium and any Part D plan premium. Social Security decides it from your modified adjusted gross income on the tax return from two years before, so 2026 premiums are set by your 2024 return.
The 2026 figures. The CMS fact sheet for 2026 puts the standard Part B premium at $202.90 a month. Above $109,000 of MAGI on an individual return, or $218,000 on a joint return, five tiers add between $81.20 and $487.00 to Part B and between $14.50 and $91.00 to Part D. The tiers are cliffs: one dollar over a bracket costs the whole step for the year, which is why the calculator shows how far you are from the next one.
Which income. MAGI for IRMAA is adjusted gross income plus tax-exempt interest, the two figures on lines 11 and 2a of Form 1040. Roth conversions, capital gains and required minimum distributions all raise AGI, so a large one-off can put you in a higher tier two years later.
Married filing separately. If you lived with your spouse at any time during the year and file separately, CMS uses a harsher table: anything over $109,000 goes straight to the fourth Part B tier. If you lived apart all year, the individual table applies.
Lowering it. If your income fell because of a life-changing event such as retirement, the death of a spouse or divorce, you can ask Social Security to use a more recent year with Form SSA-44.
What this calculator does not include: your Part D plan's own premium, which varies by plan, the immunosuppressive-drug-only Part B premium, and late-enrollment penalties. CMS publishes the 2027 amounts each autumn; the figures here are for 2026.
For an individual return, the 2026 Part B premium is $202.90 up to $109,000 of MAGI, then $284.10, $405.80, $527.50 and $649.20 at thresholds of $137,000, $171,000 and $205,000, and $689.90 from $500,000. Joint returns use double the thresholds up to $410,000 and $750,000 for the top tier. Figures are from the CMS 2026 fact sheet.
Medicare uses the modified adjusted gross income on your IRS tax return from two years earlier. For 2026 premiums, Social Security looks at your 2024 return. If that return is not yet available, it uses the one from three years before and corrects the amount once the newer return arrives.
MAGI for Medicare premiums is your adjusted gross income plus tax-exempt interest: line 11 of Form 1040 plus line 2a. Municipal bond interest therefore counts even though it is not taxed. Roth IRA withdrawals do not count, but Roth conversions, capital gains and required minimum distributions do, because they raise AGI.
The 2026 Part D surcharge is $14.50, $37.50, $60.40, $83.30 or $91.00 a month, on the same income brackets as Part B. Medicare adds it to whatever your drug plan charges, and it is usually taken from your Social Security payment, even when you pay the plan's own premium to the plan directly.
Yes. After a life-changing event such as stopping work, reduced work hours, the death of a spouse, marriage, divorce or the loss of a pension, file Form SSA-44 with Social Security and ask it to use a more recent year's income. A one-off gain such as selling a house is not a life-changing event.
CMS applies a separate table to married people who lived with their spouse at any time in the year but file separate returns. Only two thresholds apply: over $109,000 costs $649.20 a month for Part B, and $391,000 or more costs $689.90. Married people who lived apart all year use the individual table instead.
