Photograph the label straight on, with the white margin at each end included, and the number comes back along with its check digit recalculated, so a misread shows up as a mismatch rather than as a plausible wrong product. EAN-13, EAN-8, UPC-A, UPC-E, Code 128, Code 39, ITF and Codabar are all read.

Thirteen formats are read. QR is the one most people mean, and alongside it: EAN-13, EAN-8, UPC-A and UPC-E from shop shelves, Code 128 and Code 39 from warehouse and asset labels, ITF from cartons, Codabar from libraries and blood banks, Data Matrix from parts and medicine packs, PDF417 from driving licences and boarding passes, Aztec from train tickets, and GS1 DataBar from fresh produce. A product number has its check digit recalculated, so a misread shows as a mismatch rather than as a plausible wrong product.
A web address found inside a code is printed as text and is never turned into a link. That is the whole point of the page. A QR code is the only interface in ordinary use where somebody commits to a destination they cannot read, and a sticker placed over the real code on a parking meter, a restaurant table or a letter is now an everyday kind of fraud. So the things that matter are named first: a hostname written in Punycode and the characters it will actually display as, a familiar name placed in front of an @, a link shortener hiding the destination, plain http instead of https, a bare IP address, an unusual port. A UPI payment request says out loud that scanning can only send money and never receive it, which is the commonest payment scam in India.

No. The decoding runs in your browser, on your own device, with no network request. Camera frames are read and discarded in memory and never recorded. That matters most for a WiFi code, because the password is inside the pattern in plain text.

Usually the framing. Crop to the code with a little blank space around it, photograph it straight on rather than at an angle, and light it evenly. A one-dimensional barcode also needs the blank margin at each end; cropped tight against the bars, no scanner can start.
